Submitting Information

Incoming 11th grade students:

All 11th graders are required to submit a physical to the school nurse. Although a private physical performed by the family doctor is preferred since that doctor knows your child best, a sports physical, work permit physical, or driver's license physical will also be accepted. Any student who does not provide proof of physical exam will be scheduled for an exam with the school physician. Please contact the school nurse with any questions.

Incoming 12th grade students:

All 12th grade students are required to submit proof of receiving a 2nd MCV vaccine prior to the first day of school. This can be submitted via fax, email, or physically brought into the nurses office. Fax and email information is provided on the "Nurses Nook" page. Any student who does not submit proof of their second MCV vaccine by the 5th day of the start of school, will be excluded from school until proof is provided.  Parents do have the right to morally or religiously exempt their students from these vaccines. Forms to do so can be provided by the school nurse.
